If your jawbone is not strong enough to support the titanium screw, you may require bone grafting. These options can all be discussed during your initial consultation, which involves the latest in 3D scanning technology and an assessment of your medical history.

After the titanium screw is attached, your gums will need around two weeks to heal. A healing cap is applied to the implant to aid this process. Once your gums have healed, an abutment made of titanium, porcelain or gold is attached to the implant. An abutment is essentially something that joins two things at a border: in this case, your implant and the crown.

As with the surgery for titanium screws, the surgery for your abutment requires healing time for your gums, during which a temporary crown is attached. Finally, once healing has taken place, your dentist attaches the crown, typically made of a combination of porcelain and metal. This new tooth looks like all your other teeth, and you can treat it as such when brushing or flossing.

Surgery To Place Implant

In an All-On-4® dental implant procedure, a patient’s set of upper and lower teeth are restored with only four implants. These implants provide the support required to place a set of permanent teeth.
The All-On-4® dental implants procedure usually requires the patient to be under general anaesthesia as it is a surgical procedure. However, when the anaesthesia wears off, the patient might experience discomfort such as swelling and minor aches for a few days. Special painkiller medications can be prescribed to patients to alleviate pain.
The All-On-4® treatment can be recommended for anyone that needs a permanent dental restoration for multiple sets of teeth. The candidate is required to have healthy gums and bone mass to hold the implants. These may include patients that have endured tremendous tooth loss or who want a permanent, long-lasting solution to removable dentures. For such patients, an All-On-4® treatment can help restore vital facial features, align jaw position and bite force, as well as enhance speech.
With good dental hygiene, maintenance, and care, standard All-On-4® restorations can last indefinitely. However, this is very much dependant on the maintenance of the implants and prosthetics.
Since the All-On-4® implants are tilted within the jaw, the requirement for bone grafting is minimised. The positioning on both the lower and upper jaws is adjusted to ensure maximum functionality and to prevent any dental concerns like cavities from forming. For this reason, it’s necessary for the dentist to analyse the bone structure to see if they are a fit for the procedure.

Initially, the dentist starts with an examination of the patient’s overall oral health to determine whether they are the right fit for the procedure. Once the patient qualifies, the dentist proceeds with the surgery.

During a standard All-On-4® procedure, the dentist fixes a set of four implants and provisional teeth on top for a period of 3 months as the gum and bones heal. After the surgery, the mouth is left to recover while the implants integrate firmly with the jawbone.

Following the recovery, the dentist creates the permanent teeth made of either zirconia, acrylic or ceramic. These are then fixed permanently over the frame resulting in a complete restoration of the patient’s teeth.

Patients can also opt for sleep dentistry as a part of their treatment to help manage their fear or anxiety. Undergoing a treatment under sleep dentistry means the sedation is supervised by a medical professional who can monitor their condition till the procedure is complete. This is to ensure the overall safety and comfort of the patient.
It’s not usually necessary to remove both upper and lower arches in an All-On-4® treatment if one of them are in good shape. However, if both arches are badly damaged, it’s advisable to get them both replaced in a single treatment. This helps reduce cost of the whole treatment.
The completion time for an All-On-4® treatment can vary on certain factors. Ideally, the patient can expect to get a full restoration of their implants within 24 hours with their temporary teeth. Within the said time period, the patient will have their All-On-4® teeth permanently fitted into the implants. The treatment duration might, however, increase if the patient requires any add-ons to be done.
Compared to regular implants where having a firm jawbone is necessary, All-On-4® implants can be placed strategically to manage a single dental arch. This means a dentist can find an ideal foundation in the jawbone to insert the All-On-4® implants.
